Recently, a medical emergency unfolded at a restaurant around midday when a middle-aged man suddenly developed massive upper gastrointestinal bleeding accompanied by signs of shock, placing his life in immediate danger. After receiving the emergency dispatch, the ambulance team led by Jia Zhihui from the Tieying Emergency Station of the Beijing 120 Fengtai Emergency Medical Center responded rapidly and provided standardized prehospital emergency care, successfully stabilizing the patient and securing valuable time for definitive hospital treatment.

The incident occurred shortly after the man had eaten spicy food. He suddenly developed severe abdominal cramping, nausea, and retching, followed by profuse vomiting of blood and the passage of black, tarry stools. Within a short period, he experienced dizziness, palpitations, chest tightness, profound weakness, and was unable to stand. He became pale, his extremities turned cold and clammy, and his condition deteriorated rapidly. Restaurant staff immediately called 120 for emergency medical assistance.

Upon arrival, the emergency team promptly assessed the patient. A rapid physical examination and medical history revealed that he had a history of gastric ulcer and chronic gastritis and had long maintained irregular eating and sleeping habits. Based on his symptoms and clinical findings, the team made a preliminary diagnosis of acute upper gastrointestinal bleeding with signs of early hemorrhagic shock, indicating a life-threatening condition requiring immediate intervention.

Working in close coordination, the emergency personnel quickly initiated treatment. Nurse Li Yi and stretcher bearer Li Qi carefully placed the patient in the side-lying position to maintain a clear airway and reduce the risk of aspiration. Meanwhile, the team continuously monitored his vital signs, which showed hypotension and a rapid, weak pulse and significant shock. Emergency personnel immediately established intravenous access and began rapid fluid resuscitation to restore circulating blood volume and treat the developing shock.

Following a series of timely and effective prehospital interventions, the patient's hematemesis and gastrointestinal bleeding were brought under control. His dizziness and palpitations improved significantly, his vital signs gradually stabilized, and he remained conscious and alert. The patient was then carefully transferred to the ambulance for transport to the hospital. During the journey, the emergency team continued close monitoring and ongoing treatment while simultaneously communicating with the receiving hospital through the prehospital communication system. This allowed the hospital to activate its emergency "green channel". The patient’s bleeding has been successfully controlled, and his condition continues to improve.

Beijing 120 Reminder: People with underlying conditions such as gastritis, gastric ulcers, or liver cirrhosis should exercise caution when consuming spicy, extremely hot, cold, or other irritating foods. They should avoid overeating and excessive alcohol consumption, and maintain a regular, light, and balanced diet.
