At 8:10 a.m. on May 5th, shortly after the ambulance crew at Baizhifang Station of the Xicheng District Emergency Center took the shift, the 120 dispatch call hurriedly rang. A primipara gave birth to a baby boy in the dormitory at her company, and the mother and baby urgently needed to be sent to the hospital for treatment. On their way to the scene, Dr. Zhao Wei called a colleague in the maternity unit and learned that the newborn had been delivered for nearly half an hour, the placenta had been delivered, and the umbilical cord had been cut. Dr. Zhao Wei immediately called and instructed the colleagues to keep the newborn warm and observe the maternal vaginal bleeding. After the ambulance arrived at the scene, the medical staff immediately carried out various inspections for the mother and newborn. Nurse Li Yang established intravenous access, oxygen inhalation and ECG monitoring for the puerpera. The ambulance team contacted the dispatching command center and requested to establish a green channel at the nearest hospital, and finally sent the mother and newborn to the Beijing Electric Power Hospital safely. They successfully completed the handover with the obstetrics, gynecology and pediatrics medical staff. It is learned that the mother began to have paroxysmal abdominal pain that night. Due to the lack of knowledge about pregnancy and childbirth, she had no obstetric examination throughout her pregnancy. How to reduce the occurrence of similar incidents?

1. Increase security awareness

Primiparaes should strengthen safety awareness, and pay attention to the psychological state and physical condition during pregnancy and childbirth, to prepare well and reduce risks.

2. Raise awareness of obstetric inspection

Neglecting regular pre-pregnancy and prenatal examinations may result in failing to detect abnormal pregnancies in time, which will lead to a significant increase in the incidence rate of fetal malformation, fetal distress, and threatened miscarriage. It is important to have regular check-ups to monitor the development of the baby and the physical condition of the mother such as blood pressure and blood sugar.

3. Enhance labor awareness:

What symptoms indicate that the expectant mothers should go to the hospital for delivery?

Uterine contractions (labor pains). For primiparous women, intermittent uterine contractions generally last for 8-10 hours. Once the uterine contractions are frequent, severe and regular, and appear about every 5 minutes or so, and if the uterus seems to become hard, and pain or backache occur, it means that childbirth is about to begin.

Vaginal bleeding. The cervix begins to dilate during labor, when a small amount of blood mixed with mucus flows out, which is only a little bleeding. A small amount of bleeding can occur in the first few days of labor. If there is a large amount of painless vaginal bleeding or lower abdominal cramps accompanied by bleeding, there may be a risk of placenta previa or placental abruption, and it is necessary to go to the hospital immediately.

Amniorrhea (water break). The water break usually occurs when the uterus is contracting regularly, but it can happen earlier. Amniorrhea can easily cause umbilical cord prolapse, which may threaten the life of the fetus. It is necessary to go to the hospital for observation immediately after the water break even if you have no other symptoms.

120 reminds that maternal and child health is a major public health issue. The number one cause of maternal death in China is postpartum hemorrhage. It refers to the fact that during the production process, the placenta, uterus and other related organs cannot recover well, causing massive bleeding, resulting in ischemia and hypoxia, which poses a serious threat to women's life. Pregnant women should strengthen health care and health education during pregnancy, and improve the personal health awareness. It is recommended to take regular prenatal checkups, to timely detect pregnancy complications, so that the early targeted measures can effectively reduce the risk after childbirth.
